Home care in Malaysia: does it improve patients' care

Home care service is an evidence-based approach in the care for people with serious mental illness. In accordance to our National Mental Health Policy 1998 and national mental health service framework 2001, we are actively providing hospital-based outreach community psychiatric care, including components of acute home care and assertive care. Acute home is offered as an alternative to hospitalization after risk assessment. Assertive care addresses the needs of complex patients and their carers. Family intervention is seen an essential component of the multimodal approach. Our Clinical Practice Guideline on Schizophrenia (2009) recommended that Community Mental Health Team (CMHT) should be offered to patients in the community to promote greater acceptance of treatment, and is useful in people with repeated admissions. There is evidence in our country that provision of home care improves patients' care by better satisfaction among patients and carers, increase adherence to medication, reduction of relapses and hospitalization.
